The Warning Is Already in Your Data. Safety Mojo Can Find It.

See how Safety Mojo's Risk Feed ranks a week's worth of submissions, and what it means for your frontline crews.

01

The Status Quo

Your Frontline Data is Already Telling You Where the Next Incident Starts

One safety director can be responsible for three structures going up at once, 40 to 50-plus subcontractors, and thousands of workers. Getting full visibility over risk and safety is challenging. The early warning for most incidents is already sitting in what those crews submit.

Pre-Task Plan

A plan that names a hazard but lists no controls for it marks a gap before the crew starts work.

Observation

A scissor lift sharing a congested aisle, or a cord run through standing water, points straight at where the next incident could start.

Near Miss

A load that swung wide and hit nothing is the clearest warning you get without an injury.

One Program. One Week.

Get Your Top Action Items From Thousands of Submissions

That's what one Safety Mojo customer program submitted from August 19 to 25, 2026. Every one of them could hold the warning.

2,859 Observations

443 PTPs

99 Executive observations

65 Incidents

57 Good catches

12 Corrective actions

How Long Would It Take to Read Them All?

Your math. Set the volume and how long a careful read takes.

59
hours of reading per week
8.4
hours per day, seven days a week

Before anyone walks a site, coaches a crew, or closes a corrective action.

The Blind Spot

A Hazard on Paper at 7:00 a.m. Isn't Seen Until the Form Is Read

The old way runs on delayed reports, fragmented spreadsheets, and someone interpreting them after the fact. End-of-day reports are stale by the time you read them. By the time you spot the pattern, the exposure has already developed and is rolling down the tracks on its own.

Visibility, in Real Time

Meet

Live Risk Feed.

It reads every PTP, observation, and incident report your crews already submit, ranks what matters most, and tells you where to go first and what to fix when you get there.

Reads Everything

Every submission, as it comes in. Nothing new for crews to fill out.

Ranks What Matters

Severity, recent activity on the job, and your priorities. The worst exposure is at the top.

Brings the Fix

Every flag explains why, gives the best-practice fix, and links to the source.

How Live Risk Feed Works

See the Submissions That Matter Most.

Without real-time data, you’re reacting to risk. Live Risk Feed shows you the top hazards on the job site and how to fix them.

Reads every submission

3,535 Records In. 25 Insights Out.

Live Risk Feed reads PTPs, observations, incidents, near misses, good catches, corrective actions, and executive safety walks as they're submitted. Nothing new for crews to fill out.

Start the day where you’re needed the most.

Ranked by severity

See High Priority Hazards

Each submission is weighed against how serious the hazard is under OSHA standards, what's happened recently on that job, and the priorities your safety team sets.

See what’s wrong and how to fix it.

Before work starts

Potential Hazards Get Flagged

Live Risk Feed flags hazardous work planned for today, the morning the PTP comes in. This one scored 90, Critical.

Supervisors can add controls before work starts.

Unfamiliar work

Out-of-Routine Work Gets Flagged

A crew assigned to falling-tools work it hasn't planned for in the prior 14 days gets flagged, even though the PTP itself was submitted.

Support Your Crew Before Work Starts

The fix comes with the flag

See What Was Flagged and How to Fix It

Every item explains its reasoning and gives a best-practice fix: verify lockout/tagout and voltage-rated PPE, and stop the work if the safeguards can't be confirmed.

You have the info you need to act right away

Connects the dots

Five Observations. Three Days. One Pattern.

Separate observations about electrical tools and cords, logged across different crews and days, become one item with every source submission linked.

Handle the hazard before someone gets hurt.

Positive recognition

Good Work Shows Up Too

Positive callouts sit in the same feed: a contractor with six positive fall-protection observations across four days, a project at 90 days with zero incidents.

Highlight good behavior and strong engagement.

What it Means

Spend More Time In Field, and Less in the Office

See what Live Risk Feed changes for the people running the job.

Keep Your People Safe

Get Everyone Home Safe

How it is Today

A PTP for electrical work with zero controls sits in the stack until someone gets to it.

With Live Risk Feed

It's the first item on your list, before the crew starts, with the fix attached: confirm lockout/tagout, training, and voltage-rated PPE, or stop the work.

84% of contractors with seven or more pre-task planning activities report reduced recordable injury rates (Dodge & CPWR, 2023). Live Risk Feed catches the plans that fall short.

Keep Projects on Schedule

Catch It While It's Still a Conversation

Today

Problems show up in end-of-day reports that are stale by the time you read them.

With Live Risk Feed

A crew starting unfamiliar work, or a cord problem repeating across shifts, shows up the morning it's logged.

At hyperscale speed, one lost day can put a major dent in progress, especially on a critical-path item. Damaged equipment can carry a six-month lead time.

Get Your Time Back

Spend Less Time Processing Forms

Today

59 hours a week to read every record (at one minute per record).

With Live Risk Feed

The call on what matters most is already made. Send your people to the projects, and the areas, where high-energy work is happening today.

Construction teams already lose about 5.5 hours a week hunting for information they have (FMI / PlanGrid).

See the Whole Job

Built for Jobsite Leaders

Today

One report for everyone, and everyone sorts through it.

With Live Risk Feed

Foremen see their crew and work area. Superintendents see their site. Safety directors see across projects.

Scoped by role, so each person sees what's theirs to act on.

No Lift to Get Value

Runs on Data You Already Collect

If your crews are filing PTPs and observations today, the feed has something to rank today. No separate data entry, no ramp-up period.

Recognition

Your Best Crews Hear About It

Positive callouts sit in the same feed as the flags, so good work gets seen without you having to go looking for it.

04

Real-World Impact

Real-World Case Study

Two Crews. One Water Line. A $100 Million Claim.

On a large data center project, two contractors were working on the same water line. Each crew thought the other had isolated the water source. One of them cut the line. There was an MOP in place.

Water flooded the data hall and destroyed the tenant's electrical equipment, already installed, commissioned, and ready to go. Switchgear at that scale carries at least a six-month lead time, so the building couldn't open on schedule. Between replacement equipment and liquidated damages, the claim ended up north of $100 million.

It wasn't a worker injury. It wasn't an OSHA reportable.

It was a coordination and communication gap.

Incidents like this don't start as catastrophes. They start small and build on themselves, in information that never reaches the right people in time.

What Sits Behind Most Incidents

Four Exposures That Don't Announce Themselves

Trade Stacking

Too many people working in the same place at the same time. Nobody knows who has priority.

Communication Failures

The GC, subs, and owner working off different documents. Everybody thinks they know what's happening. Nobody does.

Poor Sequencing

Trades rushing ahead under schedule pressure. Mechanical starting before structural is finished.

Planning Failures

MOPs not followed. Valves not verified. Systems activated before they're confirmed safe.

As described by a safety leader with nearly 40 years in the industry, on what comes up in almost every serious incident he's seen.

1,034

construction workers died on the job in 2024, the deadliest private industry in the country.

What We Hear From Safety Leaders

"By the time I see a problem, it's already a week old."

"I have six job sites to manage and no idea which one needs me today."

"Everything on my list looks equally urgent."
